In compliance with HIPAA’s Privacy Rule and LegitScript’s healthcare standards, Gev’s Recovery has designated a Privacy Officer and provided full contact information in our Notice of Privacy Practices . Our updated Notice follows the HHS model format: we include the organization’s name, address, and the Privacy Officer’s email and phone as required by the Privacy Rule .
How We May Use and Disclose Your Health Information
We use and disclose Protected Health Information (PHI) only as necessary for treatment, payment, and health care operations. Examples include coordinating your care with other providers, obtaining payment from insurance, and running our practice. We may also use PHI for quality improvement, training, accreditation, and other operations (for example, sharing information with other providers involved in your care, billing your insurance, and managing our facility).
We may disclose PHI without your authorization for certain public purposes (such as reporting public health concerns, abuse, or emergencies) when permitted or required by law. We comply with all HIPAA regulations concerning these uses and disclosures.
Your Rights Regarding Your Health Information
You have rights under HIPAA with respect to your PHI. These include the right to:
- Access and copy your health records (we will provide them within 30 days of request, with a possible cost-based fee) .
- Request amendments if you believe the information is incorrect (we will respond within 60 days) .
- Request restrictions on disclosures (we must agree if you pay out-of-pocket in full and request non-disclosure to your insurer) .
- Confidential communications: Ask us to communicate PHI in a specific way or at a specific location (we will accommodate reasonable requests).
- Accounting of disclosures: Receive a list of disclosures of your PHI for purposes other than treatment, payment, or operations (free once per year).
- Copy of this Notice at any time, even if you agreed to receive it electronically.
- Choose a personal representative (health care proxy or medical power of attorney) to exercise these rights on your behalf, with proof of authority.
- File a complaint if you believe your privacy rights have been violated (with Gev’s Recovery or the U.S. Department of Health & Human Services Office for Civil Rights). We will not retaliate against you for filing a complaint.

Our Responsibilities and Legal Duties
Gev’s Recovery is required by law to maintain the privacy and security of your PHI . We must follow the privacy practices described in this Notice. We will notify you promptly of any breach of your unsecured PHI. We will not use or share your information except as described in this Notice (unless you specifically authorize another use). If we make material changes to our practices, we will update this Notice and post the revised version on our website.
